Working collaboratively with physicians, nurses, dietitians, and other healthcare professionals, the Clinical Pharmacist (PharmD) initiates, modifies, or discontinues medications for chronic disease management, including conditions such as hypertension, diabetes, asthma, hyperlipidemia, and anticoagulation therapy. The Clinical Pharmacist is also adept at managing acute conditions, conducting controlled substance audits, preparing sterile IV medications, and performing inpatient medication reconciliation. The role demands attention to detail in evaluating medical and medication histories, identifying and resolving drug-related issues, and ensuring prescription accuracy and regulatory compliance.
In addition to direct patient care, the Clinical Pharmacist (PharmD) is involved in clinical investigations, outcome studies, and the development of wellness programs. Providing education and training to both patients and clinical staff, the Clinical Pharmacist fosters a culture of continuous learning and quality improvement. Duties & Responsibilities
- Assess patient responses to drug therapy; plan and adjust medication regimens based on provider diagnoses.
- Order and evaluate laboratory tests to monitor and optimize therapeutic outcomes.
- Initiate, modify, and discontinue medications for chronic and acute conditions in collaboration with the medical staff.
- Monitor pharmacotherapy requiring periodic adjustment due to pharmacokinetic changes.
- Administer prescription and non-prescription medications per established protocols.
- Order therapeutic enteral or parenteral nutrition products for inpatients as needed.
- Evaluate medical and medication histories to identify and resolve drug-related problems.
- Collaborate with healthcare providers regarding patient treatment plans and medication options.
- Conduct controlled substance audits and ensure compliance with all applicable regulations.
- Prepare sterile IV medications and perform inpatient medication reconciliation.
- Lead or participate in clinical investigations and outcomes research as approved by IRB and pharmacy committees.
- Deliver patient counseling, medication education, and wellness program support.
- Review patient profiles, ensure prescription accuracy, and intervene on discrepancies.
- Compound medications and supervise pharmaceutical product preparation by technical staff.
- Maintain order verification accuracy according to national standards.
- Supervise inventory control and ensure pharmacy and controlled substance security.
