Clinical Pharmacist (PharmD) - Summary
- Deliver high-quality clinical pharmacy services in an ambulatory care setting.
- Develop, document, and manage therapeutic plans for a diverse patient population.
- Collaborate with interdisciplinary healthcare teams to ensure optimal outcomes for patients.
- Support military service members, veterans, and their families through expert medication management.
Duties & Responsibilities
- Develop and document individualized therapeutic plans for patients receiving ambulatory care services.
- Evaluate medication therapies, prescribing medications, devices, and supplies as permitted by established protocols and scope of practice.
- Order, review, and analyze laboratory tests and diagnostic studies necessary to monitor and support patient medication therapy.
- Monitor patient responses to drug therapy using pharmacy records, laboratory data, and direct patient interactions.
- Order consults to maximize drug therapy outcomes and support disease state management.
- Educate and counsel patients regarding medication therapy, adherence, and protocol requirements.
- Maintain comprehensive documentation in accordance with regulatory standards and institutional policies.

Qualifications & Requirements
- Doctor of Pharmacy (Pharm.D.) from an ACPE-accredited institution or equivalent foreign credentialing.
- Current, full, active, and unrestricted license to practice as a clinical pharmacist.
- Basic Life Support (BLS) certification.
- At least 2 years of full-time clinical pharmacy experience in an ambulatory care setting within the past 3 years, or completion of an ASHP-accredited pharmacy residency.
- One of the following: Board of Pharmacy Specialties certification, completion of a clinical pharmacy residency or fellowship (ASHP or ACCP accredited), or a post-graduate Master of Science degree with an emphasis in medication therapy management or direct patient care.
