Nurse Practitioner (NP) – Primary Care Summary
- Full-time position delivering value-based outpatient primary care to geriatric patients
- Work with a small patient panel, enabling personalized, attentive care
- Collaborate with a dedicated care team including Medical Assistants, Schedulers, Referral Specialists, Social Workers, and Pharmacists
- Engage in clinic-based care, with occasional home visits and telehealth support as needed
Duties & Responsibilities
- Conduct thorough patient assessments, physical examinations, and diagnostic screenings
- Manage chronic conditions such as diabetes, COPD, CHF, dementia, and debility
- Develop and implement individualized treatment plans that align with patient goals and best practice standards
- Educate patients and families on medications, nutrition, disease management, and preventative health strategies
- Collaborate closely with an interdisciplinary team to ensure coordinated, seamless care
- Prescribe medications in accordance with state scope of practice
- Deliver care in various settings, including the clinic, patient residences, and via telehealth technology
- Maintain detailed and thorough documentation that supports risk-adjustment and quality metrics

Qualifications & Requirements
- Master's degree in Nursing (MSN) from an accredited program
- Board-certified Nurse Practitioner (Adult, Adult-Gero, or Family)
- At least 2 years of outpatient primary care experience (required)
- Experience with value-based care models (preferred)
- Demonstrated ability to practice autonomously and make independent clinical decisions
- Active Nurse Practitioner licensure for the state of practice
- DEA certification or eligibility
- Proficiency in managing geriatric patients, chronic disease management, wound care, injections, ABI, or imaging interpretation (preferred)
- Experience with Electronic Medical Records (EMR); Athena EMR experience highly preferred
- No new graduates
