Nurse Practitioner (NP) – Primary Care Summary
- Remote, telehealth-based Nurse Practitioner (NP) position in primary care delivery

Duties & Responsibilities
- Conduct comprehensive virtual patient assessments using telemedicine platforms
- Develop and implement individualized patient care plans based on clinical best practices
- Manage chronic health conditions, including diabetes, hypertension, and cardiovascular diseases
- Provide preventive health services and education on disease prevention and wellness
- Deliver palliative care support and guidance to patients and their families as required
- Order, interpret, and review diagnostic tests to inform clinical decision-making
- Prescribe medications and treatments in accordance with Nevada licensure and scope of practice
- Document all patient encounters accurately and promptly within the EMR system
- Maintain professional, effective communication with patients, families, and facility staff
- Navigate multiple facility partnerships and foster strong, collaborative clinical relationships
- Participate in collaborative care with primary care physicians and other healthcare professionals
- Promote medication adherence, healthy lifestyle changes, and patient engagement

Qualifications & Requirements
- Master’s degree in Nursing (MSN) from an accredited institution
- Current, unrestricted Registered Nurse (RN) and Nurse Practitioner (NP) licenses in Nevada
- Minimum of one year of experience as a Nurse Practitioner (NP), preferably in primary care, geriatrics, or palliative care
- Telemedicine experience preferred but not required
- Strong clinical judgment, communication skills, and ability to build rapport with patients
- Demonstrated ability to work independently while collaborating with a team
- Experience managing chronic conditions in adult or geriatric populations is advantageous
