Nurse Practitioner (NP) – Primary Care Summary

Duties & Responsibilities
- Provide primary care services to a panel of up to 450 patients, predominantly seniors with 3-5 chronic conditions
- Conduct comprehensive patient assessments, develop individualized care plans, and manage chronic and acute illnesses
- See approximately 18-20 patients per day
- Maintain Monday-Friday schedule, with clinic hours from 7:30am to 5:00pm (last patient at 4:15pm); no nights or weekends
- Coordinate closely with an assigned care team, including a medical assistant, scheduler, and referral specialist
- Collaborate with in-house consulting specialists (Cardiology, Podiatry, and more) as needed for patient care
- Integrate holistic health services such as acupuncture, Tai Chi, yoga, and nutrition into patient care plans
- Document care accurately and efficiently, with administrative time built into your schedule
- Participate in quality improvement initiatives and value-based care programs

Qualifications & Requirements
- Board Certified Nurse Practitioner (NP)
- Minimum of 2 years of outpatient primary care experience as a Nurse Practitioner
- Experience working with geriatric populations is preferred
- Commitment to value-based, patient-centered care
- Excellent communication, organizational, and teamwork skills
- Ability to thrive in a fast-paced, diverse clinical environment
