Physical Therapist Assistant (PTA) Home Health - Summary
- Deliver direct physical therapy services to patients in their homes, following the care plan established by the supervising Physical Therapist.
- Support patients in achieving their therapy goals through evidence-based interventions and patient education.
- Serve as a vital liaison between patients, families, and the wider clinical team to ensure optimal care coordination and communication.
- Maintain accurate clinical documentation and adhere to organizational and regulatory standards.
Duties & Responsibilities
- Promote Elara Caring’s philosophy and mission, ensuring quality care and compliance with administrative policies.
- Administer prescribed physical therapy interventions under the supervision of a licensed Physical Therapist.
- Conduct patient assessments and report any changes in patient status to the supervising Physical Therapist.
- Follow the Plan of Care and State Practice Act Guidelines at all times.
- Verify the Plan of Care prior to each visit and deliver care in accordance with physician orders, assessment data, and established guidelines.
- Incorporate patient care goals into therapy interventions, adjusting as needed based on patient progress.
- Educate patients and families regarding the safe use of prosthetic, orthotic, and assistive devices such as canes, walkers, and wheelchairs.
- Train patients and caregivers on the home therapy program to encourage independence and continuity of care.
- Demonstrate the ability to manage the physical demands of the role, including patient transfers and equipment handling.
- Adhere to scheduled visits, which may include irregular hours or call coverage as required.

Qualifications & Requirements
- Graduate of an accredited Physical Therapist Assistant program recognized by the American Therapy Association.
- Current, unrestricted PTA license valid in the state of practice.
- At least 6 months of experience as a Physical Therapist Assistant preferred.
- Willingness and ability to travel within the assigned branch or office coverage area.
- Dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.
- Ability to frequently lift, push, pull, and support up to 50 pounds, including patient positioning and equipment movement.
