- Develop individualized, evidence-based care plans that align with patient goals and clinical best practices.
- Work closely with other healthcare professionals to coordinate care and support patients throughout their rehabilitation journey.
- Maintain professional standards, ethical practices, and regulatory compliance in all aspects of care delivery and documentation.
- Serve as a clinical resource and mentor to support staff and contribute to community education initiatives as needed.
- Deliver direct patient care by assessing, planning, and implementing physical therapy interventions in the home health setting.

- Graduate of an American Physical Therapy Association (APTA)-accredited physical therapy program.
- Active, unrestricted Physical Therapist (PT) license in the state of practice.
- Minimum of one year of professional experience in a healthcare facility, preferably with exposure to home health or rehabilitative care.
- Proven ability to travel within assigned coverage areas, with access to a reliable vehicle, valid driver’s license, and current auto insurance.
- Physical capability to lift or transfer patients and equipment (up to 50 pounds).
