Physical Therapist (PT) – Home Health Summary
As a Physical Therapist (PT) – Home Health at Elara Caring, you will be responsible for providing direct, one-on-one care to patients in the comfort of their own homes. Duties & Responsibilities
- Deliver direct physical therapy services to patients in their homes, ensuring patient safety and comfort at all times.
- Evaluate patients’ functional abilities and therapeutic needs through thorough assessments.
- Develop and implement individualized care plans based on assessment results, incorporating input from patients, families, and caregivers.
- Monitor and document patient progress, adapting treatment plans as necessary to achieve targeted outcomes.
- Provide education and support to patients and their families regarding therapy goals, techniques, and protocols for home-based exercises and activities.
- Consult with physicians, nurses, and other healthcare professionals to coordinate patient care and achieve interdisciplinary treatment goals.
- Guide and supervise support staff in the execution of therapy plans and promote best practices in patient care.
- Maintain accurate and timely documentation, including evaluations, treatment goals, plans of care, and progress notes, in accordance with regulatory and organizational standards.
- Serve as a knowledgeable resource to promote the benefits and scope of physical therapy within the community.
- Adhere to established standards of care, ethical guidelines, and policies relevant to home health physical therapy practice.

Qualifications & Requirements
- Graduate of a physical therapy program accredited by the American Physical Therapy Association (APTA).
- Current, unrestricted Physical Therapist (PT) license in the state of practice.
- Minimum of one year experience working in a healthcare facility, preferably with exposure to home health or rehabilitation settings.
- Ability and willingness to travel within the branch coverage area as required by patient caseload.
- Valid driver’s license, reliable transportation, and current auto insurance.
- Physical ability to lift, push, pull, and support up to 50 pounds, including assisting with patient and equipment transfers.
- Strong clinical assessment, planning, and organizational skills.
- Excellent verbal and written communication skills for effective collaboration and documentation.
