Duties & Responsibilities

- Design and implement individualized treatment plans based on in-depth assessments, fostering active participation from patients and, when suitable, their significant others.
- Maintain clinical excellence by practicing within established standards of care and utilizing evidence-based physical therapy interventions.
- Lead and direct the efforts of support staff to effectively achieve patients’ functional and therapeutic goals.
- Document initial evaluations, treatment goals, care plans, progress notes, and maintain all required prescriptions and signatures.
- Regularly update clinical documentation to reflect patient progress and modify care plans as needed for optimal outcomes.
- Provide direction, mentorship, and leadership to physical therapy staff, acting as a resource and advocate for best practices within the community.
- Work within a physically demanding environment, including tasks such as handling, lifting, and transferring patients safely and effectively.

Qualifications & Requirements
- Graduation from an American Physical Therapy Association (APTA) approved physical therapy program.
- Current, unrestricted Physical Therapist (PT) license valid for the state of employment.
- At least one year of relevant experience in a healthcare facility, with a preference for candidates with home health experience.
- Ability and willingness to travel within the designated branch or office coverage area (up to 50%).
- Must possess a dependable vehicle, a valid driver’s license, and current auto insurance as required by state law.
