Physical Therapist (PT) - Home Health Summary- Deliver individualized physical therapy services to patients in their homes, following evidence-based best practices.
- Assess patients' functional abilities, limitations, and rehabilitation needs through thorough evaluations.
- Develop, implement, and revise personalized plans of care to achieve patient-centered goals.
- Collaborate with an interdisciplinary healthcare team to support continuity of care and maximize patient outcomes.
- Demonstrate a commitment to quality, safety, and patient advocacy in every aspect of service delivery.
- Perform direct patient care, including assessment, treatment, and re-evaluation of functional needs and progress.

- Graduate of an American Physical Therapy Association (APTA) accredited physical therapy program.
- Current, unrestricted Physical Therapist license in the state of practice.
- At least one year of experience working in a healthcare facility, with experience in home health preferred.
- Valid driver’s license, dependable vehicle, and current automobile insurance.
- Willingness and ability to travel within the assigned coverage area (up to 50%).
- Ability to frequently lift, push, pull, or support up to 50 pounds as part of patient care responsibilities.
