Physical Therapist (PT) Home Health - Summary- Deliver evidence-based physical therapy services to patients in a home health setting.
- Evaluate patient conditions, develop individualized care plans, and adjust interventions as needed to achieve optimal outcomes.
- Work collaboratively with interdisciplinary teams, patients, and their families to ensure the highest quality of care.
- Promote the mission and philosophy of Elara Caring through compassionate service and professional integrity.
- Provide direct patient care in the home, assessing functional needs and setting measurable treatment goals.
- Develop, implement, and modify individualized plans of care to maximize patient independence.
- Document all evaluations, treatment goals, progress notes, and discharge plans according to company and regulatory standards.
- Consult with other healthcare professionals, including physicians and nursing staff, to coordinate comprehensive patient care.
- Incorporate input from significant others and caregivers into therapy plans when appropriate.

- Graduation from an American Physical Therapy Association (APTA) accredited physical therapy program.
- Current, unrestricted Physical Therapist (PT) license in the state of practice.
- Minimum of one year of experience as a Physical Therapist in a healthcare setting.
- Reliable vehicle, valid driver’s license, and current auto insurance.
- Ability and willingness to travel within the designated branch coverage area, up to 50% of the time.
- Physical ability to lift, push, pull, and support up to 50 pounds.
- Demonstrates strong clinical skills and a commitment to evidence-based practice.
- Excellent communication and interpersonal abilities for working with diverse populations.
- Strong organizational skills, attention to detail, and the ability to adapt care plans as needed.
