Physical Therapist (PT) – Home Health Summary

Qualifications & Requirements
- Graduate of a Commission on Accreditation in Physical Therapy Education (CAPTE)-accredited program.
- Current, unrestricted Physical Therapist (PT) license in the state of practice.
- At least one (1) year of professional experience in a healthcare setting, preferably in home health or similar environment.
- Ability and willingness to travel throughout the designated coverage area as needed (up to 50%).
- Reliable transportation, valid driver’s license, and current auto insurance required.
- Physical ability to lift 50–100 lbs as needed when providing care and assistance.
