Physical Therapist (PT) - Home Health Summary- Deliver patient-centered physical therapy services in a home health environment.
- Assess functional needs and develop individualized treatment plans to improve patient outcomes.
- Collaborate with interdisciplinary teams to provide comprehensive care.
- Support the mission of Elara Caring by promoting high-quality, compassionate care in the home setting.
- Provide direct patient care and evaluate functional needs and outcomes.
- Design and implement evidence-based plans of care, incorporating input from significant others as appropriate.
- Document evaluations, treatment goals, and care plans in compliance with industry standards and regulations.
- Direct, mentor, and support staff to achieve optimal patient functional outcomes.
- Act as a clinical resource and promote physical therapy services within the community.
- Maintain accurate records, including prescriptions and signatures, as required by policy and law.
- Work in physically demanding situations, including lifting and supporting patients up to 50 pounds.
- Travel within the designated branch coverage area as needed.

- Graduate of an American Physical Therapy Association (APTA) accredited physical therapy program.
- Current, unrestricted Physical Therapist (PT) license for the state of practice.
- Minimum of one (1) year of experience in a healthcare facility, preferably in home health.
- Dependable vehicle, valid driver’s license, and auto insurance in compliance with state laws.
- Willingness and ability to travel up to 50% within the branch coverage area.
- Demonstrates a compassionate, patient-centered approach to care.
- Excellent communication and interpersonal skills, with the ability to collaborate effectively with patients, families, and healthcare team members.
