Physical Therapist (PT) Home Health - Summary
As a Physical Therapist (PT) Home Health, you will be entrusted with the responsibility of providing direct patient care in a home health setting. Your primary focus will be on evaluating patient needs, developing individualized care plans, and working collaboratively with interdisciplinary teams to ensure optimal patient outcomes. Duties & Responsibilities
- Promote the organization's philosophy, mission, and administrative policies to ensure the delivery of quality home health care.
- Provide direct patient care by evaluating functional needs, setting treatment goals, and implementing evidence-based interventions.
- Design and implement individualized care plans based on comprehensive assessments, integrating input from patients and their families when appropriate.
- Collaborate with physicians, nurses, and other healthcare professionals to coordinate care and achieve optimal patient outcomes.
- Adjust care plans as necessary to respond to changes in patient condition, progress, and feedback.
- Maintain accurate, up-to-date documentation of evaluations, treatment interventions, progress notes, and any adjustments to care plans.
- Ensure all patient records are compliant with organizational policies, regulatory standards, and legal requirements.
- Direct and supervise support staff to accomplish patient functional goals efficiently and safely.
- Serve as a resource and leader for staff, offering guidance and promoting best practices in home health physical therapy.

- Perform physically demanding tasks such as patient transfers and lifting, as required to assist patient mobility and safety.

Qualifications & Requirements
- Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
- Current, unrestricted Physical Therapist (PT) license valid in the state of practice.
- At least one (1) year of experience in a healthcare facility, preferably in a home health or rehabilitation setting.
- Demonstrated ability to travel as needed within the assigned coverage area, up to 50% of the time.
- Reliable vehicle, valid driver’s license, and current auto insurance in accordance with state regulations.
- Strong communication skills, both verbal and written, for effective patient documentation and collaboration.
- Ability to work independently and as part of a multidisciplinary team.
