As a Physical Therapist (PT) – Home Health, your responsibilities encompass the full spectrum of patient care, from initial evaluation through discharge planning. You will be responsible for conducting thorough assessments of each patient's functional abilities, limitations, and goals within their home environment. Collaboration with the interdisciplinary team—including nurses, occupational therapists, speech-language pathologists, and social workers—is essential to ensure a holistic approach to care and optimal patient outcomes. You will use your clinical expertise to design and implement individualized Plans of Care tailored to each patient’s unique needs, regularly reviewing and adjusting these plans to reflect progress and changing circumstances.
In this role, you will also provide guidance and direction to other clinical staff, support personnel, and caregivers involved in the patient’s therapy. Accurate and timely documentation is a critical component of the position; you will complete comprehensive evaluations, develop treatment plans, and maintain progress notes in accordance with clinical standards and regulatory requirements. Qualifications & Requirements
- Graduate of an APTA-accredited Physical Therapy program.
- Current and unrestricted state license to practice as a Physical Therapist (PT).
- Minimum of one (1) year of experience in a healthcare setting, preferably in home health or similar environment.
- Reliable transportation, valid driver’s license, and current auto insurance coverage.
- Ability and willingness to travel throughout the assigned branch coverage area as needed.
- Physical ability to lift and transfer 50–100 pounds and to work in a physically active, home-based environment.
