Primary Care Physician (Internal Medicine / Family Medicine, VBC) – Summary
- Deliver comprehensive primary care to adults 65+ in a value-based care model

Duties & Responsibilities
- Provide direct primary medical care to a panel of adult patients, majority aged 65+, with an emphasis on preventive and chronic disease care
- Conduct thorough patient assessments, develop care plans, and coordinate with integrated care team members, including nurse practitioners/physician assistants, medical assistants, social workers, and pharmacy/lab staff
- Maintain a daily patient census of 12–15, allowing for 20–40 minute visit times to foster strong patient-physician relationships
- Utilize value-based care principles to achieve optimal health outcomes
- Participate in interdisciplinary team meetings to review patient progress and ensure coordinated care
- Engage in ongoing training and professional development, including a comprehensive onboarding program focused on value-based care, documentation best practices, EMR (Athenacurrently in transition), and integrated care workflows
- Contribute to a culture that values collaboration, communication, and continuous improvement

Qualifications & Requirements
- Board Certified or Board Eligible in Internal Medicine or Family Medicine
- Experience in geriatrics preferred
- Familiarity with HEDIS measures and value-based care a plus, though not required (training provided)

- Excellent communication skills and a collaborative approach to patient management
