We are seeking a dedicated Primary Care Physician (Internal Medicine or Family Medicine, Value-Based Care, Geriatrics, MD/DO) to join a dynamic, innovative healthcare team in Nashville, TN. As a Primary Care Physician (Internal Medicine or Family Medicine, Value-Based Care, Geriatrics, MD/DO), you will work within a collaborative, multidisciplinary team that includes nurse practitioners, physician assistants, medical assistants, social workers, and lab and pharmacy professionals. Our integrated care model is designed to support you in delivering comprehensive, patient-centered care. The patient panel primarily consists of adults aged 65 and older, with a focus on both chronic disease management and preventive health measures. Physicians typically see 12 to 15 patients per day, with extended visit times of 20 to 40 minutes to ensure thorough, attentive care.
This full-time opportunity is ideal for board certified or board eligible physicians in Internal Medicine or Family Medicine. While prior experience in geriatrics or value-based care is highly desirable, it is not a requirement—comprehensive onboarding and ongoing professional development are provided. Our robust training program includes a three-week physician learning journey covering value-based care principles, documentation best practices, electronic medical record optimization, and integrated care workflows.
