We are seeking a committed and patient-focused Primary Care Physician (MD/DO) to join our innovative network of neighborhood primary care centers, serving the valued Medicare patient population in Gary, IN. Primary Care Physician (MD/DO) – Summary
- Deliver value-based, comprehensive primary care to Medicare patient populations.
- Work within an innovative care model supported by a multidisciplinary team.
- Utilize advanced EMR systems (Canopy, Greenway) and onsite laboratory resources.
- Focus on preventive care, patient engagement, and overall health outcomes.
Duties & Responsibilities
- Provide high-quality, patient-centered medical care utilizing a value-based approach.
- Manage a daily patient volume of 13-16 individuals, with regular visits scheduled for 20 minutes and full diagnostic or new patient visits for 40 minutes.
- Collaborate with a care team including scribes, medical assistants, and social workers to ensure optimal patient outcomes and support.
- Utilize onsite laboratory services, phlebotomist, and access to specialist consultations via RubiconMD.
- Participate in patient outreach and engagement activities, including community room events such as exercise classes, yoga, dance, and wellness days.
- Ensure accurate documentation and efficient workflow using Canopy and Greenway EMR systems.
- Contribute to the reduction of hospital admissions through preventive care, patient education, and evidence-based protocols.
- Maintain compliance with all healthcare regulations and uphold the highest standards of ethical practice.
- Collaborate with 3-5 providers per clinic (mix of Nurse Practitioners and Primary Care Physicians) to enhance patient care delivery.

Qualifications & Requirements
- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree from an accredited institution.
- Board Certified or Board Eligible in Internal Medicine or Family Medicine.
- Active, unrestricted, and non-probationary state medical license.
- Fellowship training in Geriatrics preferred but not required.
- No current medical residents; seeking fully credentialed practitioners.
