Primary Care Physician (PCP) – Summary- Deliver comprehensive primary care to an assigned panel of up to 450 patients
- Focus on seniors with multiple chronic conditions in an outpatient setting
- Collaborate with a robust, dedicated care support team
- Practice medicine in a value-based care environment
- Work in a physician-led organization committed to quality and innovation
- Conduct thorough patient assessments and provide ongoing management of chronic diseases
- See an average of 18–20 patients per day with adequate time allocated for each visit
- Develop personalized care plans to address the unique needs of each patient
- Lead a multidisciplinary team, including medical assistants, schedulers, and referral specialists
- Utilize value-based care strategies to improve patient health outcomes and reduce costs
- Participate in holistic health offerings, such as acupuncture, tai chi, yoga, and nutrition counseling
- Engage with consulting specialists in cardiology, podiatry, and more within the clinic
- Allocate dedicated administrative time within your schedule
- Maintain up-to-date, accurate medical records in compliance with all legal and ethical standards
- Work within a 100% outpatient clinic with no nights or weekend shifts
- Competitive base salary with incentive bonus potential
- Annual compensation package exceeding $320,000 in the first year
- Signing bonus up to $50,000

- Strong clinical skills and experience managing complex chronic disease in senior populations preferred
- Demonstrated commitment to value-based, patient-centered care
- Excellent communication and interpersonal skills
- Ability to work effectively in a collaborative, multidisciplinary team environment
- Current medical license in good standing
- Passionate about providing compassionate, high-quality care to underserved senior populations
- Adaptable, flexible, and committed to continuous improvement and innovation
- Comfortable with a smaller patient panel and able to invest time in building patient relationships
- Skilled in managing multiple chronic conditions and developing individualized care plans
- Enjoys working in a physician-led, team-based care model
