Primary Care Physician (PCP) – Summary
- Deliver comprehensive primary care to an assigned panel of up to 450 patients
- Focus on seniors with multiple chronic conditions in an outpatient setting
- Collaborate with a robust, dedicated care support team

Duties & Responsibilities
- Conduct thorough patient assessments and provide ongoing management of chronic diseases
- See an average of 18–20 patients per day with adequate time allocated for each visit
- Develop personalized care plans to address the unique needs of each patient
- Lead a multidisciplinary team, including medical assistants, schedulers, and referral specialists
- Utilize value-based care strategies to improve patient health outcomes and reduce costs
- Participate in holistic health offerings, such as acupuncture, tai chi, yoga, and nutrition counseling
- Engage with consulting specialists in cardiology, podiatry, and more within the clinic
- Allocate dedicated administrative time within your schedule
- Maintain up-to-date, accurate medical records in compliance with all legal and ethical standards
- Work within a 100% outpatient clinic with no nights or weekend shifts
Salary & Benefits
- Competitive base salary with incentive bonus potential
- Annual compensation package exceeding $320,000 in the first year
- Signing bonus up to $50,000

Qualifications & Requirements
- Completion of an accredited residency program in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine
- Board Certified or Board Eligible in IM, FM, or Geriatric Medicine
- Strong clinical skills and experience managing complex chronic disease in senior populations preferred
- Demonstrated commitment to value-based, patient-centered care
- Excellent communication and interpersonal skills
- Ability to work effectively in a collaborative, multidisciplinary team environment
- Current medical license in good standing
