Primary Care Physician (PCP) – Outpatient | Value-Based Care
We are seeking dedicated and compassionate Primary Care Physicians (PCP) to join our innovative outpatient team operating under a value-based care model. This full-time role is focused exclusively on delivering high-quality primary care to the Medicare Advantage population, ensuring patients receive comprehensive, relationship-driven medical attention. In this outpatient setting, you will manage a patient panel with a manageable daily volume, allowing for longer appointment times that foster deeper patient-physician relationships and enable you to provide thorough, holistic care. Primary Care Physician (PCP) – Summary- Outpatient role focused on comprehensive primary care for the Medicare Advantage population
- Team-based, relationship-driven approach with a focus on quality care
- Value-based care model prioritizing patient outcomes and satisfaction
- Provide 100% outpatient primary care services to adult and senior patients
- Manage a patient panel with an average daily volume of 14–16 patients
- Conduct longer, in-depth appointments to support preventive and chronic care management
- Collaborate with a dedicated care team, including medical assistants, schedulers, behavioral health professionals, social workers, and RN support
- Utilize a holistic approach to patient care, including integration with on-site fitness, social, and wellness programs
- Participate in value-based initiatives and quality improvement processes focused on metrics such as HEDIS and patient satisfaction
- Engage in ongoing professional development and team meetings
- Competitive base salary (negotiable), with potential for total first-year compensation to exceed $300,000
- Performance-based bonus structure, up to 20% of base salary, based on attainable quality and satisfaction metrics
- Sign-on bonus up to $50,000 paid within 30 days of start

- Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree
- Board certification or eligibility in Family Medicine or Internal Medicine
- Unrestricted and active medical license (or ability to obtain)

- Experience with Medicare Advantage population is a plus
