Registered Nurse (RN) – Family Health Summary
- Coordinate and deliver patient-centered care as part of a multidisciplinary team.
- Provide nursing assessments, health education, and direct patient interventions in a family health setting.
- Supervise and support healthcare staff to optimize patient outcomes and maintain high standards of care.

Duties & Responsibilities
- Coordinate patient care with a diverse team of healthcare professionals to achieve clinical, quality, and cost-effective outcomes.
- Conduct comprehensive physical exams and obtain detailed health histories to support accurate diagnoses and care planning.
- Administer medications, perform wound care, and provide other individualized medical interventions as required.
- Offer health promotion, counseling, and educational resources to patients and families, supporting informed health decisions.
- Supervise and direct the care provided by other healthcare professionals, ensuring tasks are assigned appropriately by skill level and scope of practice.
- Respond rapidly and effectively in emergency situations, recognizing adverse signs and symptoms and initiating necessary interventions.
- Communicate and collaborate with patients, families, and healthcare team members to coordinate care plans and improve patient outcomes.
- Arrange referral appointments and coordinate specialty care services where appropriate.
- Participate in ongoing research and quality improvement initiatives to advance nursing practice and patient care outcomes.

Qualifications & Requirements
- Associate Degree in Nursing from an ACEN or CCNE accredited program.
- Current, active, and unrestricted Registered Nurse (RN) license in any US state or jurisdiction.
- Basic Life Support (BLS) certification.
- Advanced Cardiac Life Support (ACLS) certification.
- At least 36 months of clinical nursing experience within the last 48 months.
