As the Registered Nurse (RN) – Special Needs Coordinator, you will oversee and manage the Exceptional Family Member Program Medical (EFMP-M) in accordance with Department of Defense and Air Force policies. Your role is critical in ensuring timely identification, enrollment, data management, and ongoing support for Air Force sponsors and their family members who have special medical or educational needs. You will use your clinical skills, leadership, and collaborative approach to ensure that all program requirements are met, while fostering a supportive and inclusive environment for families who rely on the EFMP-M program.

Registered Nurse (RN) – Special Needs Coordinator Summary
- Directly manages the Exceptional Family Member Program Medical (EFMP-M) for military families in accordance with DoD and Air Force policies.
- Ensures timely identification, enrollment, and ongoing support for family members with special medical and educational needs.
- Serves as a key liaison between families, medical staff, and community resources to coordinate comprehensive care.
- Maintains program data, provides staff training, and supports the overall mission of Decypher.
Duties & Responsibilities
- Oversee and manage the EFMP-M program, ensuring compliance with all relevant policies and guidance.
- Develop and maintain procedures to identify family members with special medical or educational needs promptly.
- Utilize clinical assessment skills to evaluate sponsors and family members for program eligibility and needs.
- Collaborate with other service providers, including youth and childcare facilities, and spouse groups, to enhance outreach and identification of special needs.
- Ensure accurate data management and annual updates for all enrolled families and sponsors in medical special needs data systems.
- Provide training and consultation to installation and medical staff to support EFMP-M implementation.
- Coordinate the Family Member Relocation Clearance (FMRC) process and oversee travel screening for families assigned to overseas bases or new assignments.
- Maintain SN files at the Medical Treatment Facility (MTF) and ensure appropriate annual contact and follow-up with enrolled sponsors.
- Work closely with the Integrated Delivery System (IDS) and other interagency forums to provide referrals and additional support to families.
- Participate in EFMP weekly care management reviews to assess newly identified families and determine necessary resources.
- Maintain positive working relationships with base personnel offices, supporting integration of EFMP-M, EFMP-FS, and EFMP-A services.

Qualifications & Requirements
- Bachelor of Science in Nursing (BSN) from an accredited institution.
- Current, full, active, and unrestricted license as a Registered Nurse (RN).
- Basic Life Support (BLS) certification is required.
- CPR/AED/First Aid certification.
- Minimum of 1 year full-time experience in care coordination, discharge planning, or case management in a medical setting within the last 4 years.
- U.S. Citizenship is required for this position.

Other Relevant Information
